I hereby make application for membership with the Rising Son MMA, LLC, and upon acceptance I sincerely pledge to obey all the rules and regulations that are set for the purpose of keeping order and maintaining discipline in the Rising Son MMA, LLC, and for the protection of its members and staff from injury. I acknowledge that such activities (and the gathering of people in public generally) include many risks, known and unknown, and that I hereby accept and assume all risks associated with such activity. I clearly recognize and accept that a risk is involved in the study of self-defense which requires strict adherence to these rules and regulations, which have been completely understood by me, and my parents or guardians (where applicable), or any beneficiaries. Anyone under the age of 18 will require acceptance of these terms by a parent or guardian as well. In consideration of accepting my application for entry into the Rising Son MMA, LLC, I hereby release all Owners, Head Instructors, all other Instructors, Members, Authorized Guests, and Affiliated Organizations from all responsibility and all claims for injuries I may receive while practicing or as a result of practicing martial art techniques, parents or guardians (where applicable), or beneficiaries of the applicant request that the application be accepted, and in consideration agree to indemnify and hold harmless and release the Rising Son MMA, LLC, and staff of the Rising Son MMA, LLC, Owners, Head Instructors, all other Instructors, Members, Authorized Guests, and Affiliated Organizations from all responsibility and all claims for injuries made on behalf of the injured party. In consideration of accepting my application for entry into the Rising Son MMA, LLC, I and/or my parents or guardians (where applicable) or beneficiaries of the applicant hereby release and hold harmless all institutions that have provided space for the purpose of practice, demonstrations or any other activities pertaining to martial arts as taught by the Rising Son MMA, LLC. In addition, it is agreed in all cases that the Rising Son MMA, LLC, will be indemnified and held harmless from any claims made by me, or on behalf of myself by my parents or guardians (where applicable) or beneficiaries for any injuries sustained while attempting to perform any self-defense act outside the domain of the Rising Son MMA setting.
